State of the Organization – July 2025

Hey Team,

We’re halfway through 2025 and making meaningful progress across all areas of GSCC. Here’s a quick, high-level look at where we are and where we’re headed:

Patient Care & Quality

  • We’re building out our Care Management Model—starting with CM-1 (basic care coordination for all patients) and gearing up to launch CM-2 and CM-3 for more complex needs.

  • We’re piloting an AI scribe tool to support providers with documentation and improve patient flow. Early signs are promising!

  • Huge win: We maintained both PCMH and FTCA recognition!

Workforce Development

  • Welcoming new providers this summer, including Dr. Rebekah Fraley, DO and APRN Megan Powers.

  • Dr. Kimbrough will be offering advanced gynecological procedures, namely colposcopies, very soon.

  • Anita Alaniz, APRN is officially a GSCC employee rather than a contract provider effective today!

  • Key roles being posted soon include a Admin Assistant, and Quality Outcomes Analyst or whatever title best suits our data analyst needs moving forward. :).

  • Current team size: 110 team members strong!

Operations & Access

  • We split the Care Coordinator and Referral Coordinator roles to better support panel management, care coordination and patient outreach as well as closing important referral loops.

  • We expanded to an all-day walk-in clinic at 12th Street—a big step toward more convenient care for our patients.

Finance & Facilities

  • We continue to navigate payment issues with Oklahoma Complete Health (a SoonerSelect/Managed Medicaid plan), and have reached out to the plan’s leadership for help in resolving the issue. As of today, they owe the GSCC over $100,000 in underpaid claims payments.

  • We’re bringing in a top-notch consultant to assess and help renegotiate our payer contracts as many of them are seriously underpaying us for the comprehensive services that we provide our patients.

  • We leased a small admin office at 7 A Street NW, downtown to support on-site staff who can’t work remotely as well as open up additional flex space for those who need it.

Community Engagement

  • 117 sports physicals provided for Lone Grove students—more school partnerships coming soon!

  • Several team members also pitched in at the local soup kitchen. ❤️

  • EmpowHER is coming up quick! Mark your schedules to volunteer with the Bloom team on September 6th from 9a to 3p.

Performance Snapshot

  • Pharmacy capture rate is climbing (now at 46.4%—goal is 50%). You can help by sharing the benefits of using our pharmacy to our patients.

  • Patient survey scores are stellar: 93.8% overall satisfaction!

    • Top praises: Team friendliness, appointment availability, and convenience.

  • Schedule utilization is improving across sites but various strategies have been and will continue to be developed to help fill the gaps.

  • 14 out of 19 UDS quality metrics were met in June—strong performance in diabetes, hypertension, depression screening, and breast cancer screening.

Compliance & Risk

  • No patient grievances filed in June—a great sign.

  • Credentialing and privileging went smoothly for all new and recredentialed team members.

  • FTCA deeming complete (yay!) but some reporting adjustments are needed for next year.

  • Q2 Risk Assessment on HIPAA and safety is wrapping up.
