Survey Says: Your Well-Being Matters to Us!

I hope this message finds you in good health and high spirits. At the heart of our new Wellness Initiative is the belief that a healthy team begins with healthy individuals. By investing in your well-being, we aim to empower each team member to thrive both personally and professionally. Investing in team health isn't just for the present; it's an investment in our future. By cultivating a culture of wellness, we're laying the groundwork for sustained success, increased creativity, and a team that stands the test of time. As part of our ongoing commitment to fostering a supportive and healthy work environment, we are excited to introduce our team experience survey focused on a wellness initiative.

1. Education is our #1 Priority:

Before we embark on this wellness journey together, understanding where we currently stand is crucial. The survey is designed to gauge your current level of wellness awareness so that we can tailor future initiatives to meet your needs.

2. Voluntary and Empowering:

Participation in the survey is entirely voluntary, and you are in complete control. You are welcome to choose not to fill it out, skip any sections that make you uncomfortable, or opt out at any time. Your responses are yours alone, and you can trust that they will be treated with the utmost confidentiality.

If for any reason, in the future you would no longer like your responses to be used in our Wellness program, you can ask for them to be removed/deleted permanently.

3. Security Measures:

To ensure the security of your information:

  • You will use your eSelfServe login ID to fill out the survey.

  • Only HR has access to your eSelfServe ID, and Dr. J, the data custodian, will not know the survey respondents' identities.

  • If you have changed your eSelfServe ID that has a unique identifiers such as your name, and you would like to participate, please let me know and I can assist you in changing your ID to a random number!

  • HR, the keeper of the ID numbers, will not have access to the survey results, eliminating any chance of connecting your information back to you.

  • Jordan, the survey creator, has no access to your ID, ensuring your responses remain anonymous.

The reason for using your eSelfServe ID is to be able to track your progress and effectiveness of the Wellness Program

4. Job Security Assurance:

Rest assured, your position or job security is entirely unrelated to your survey responses. You cannot and will not face any consequences for your honest and open feedback.

5. Support and Follow-Up:

Dr. J will be available to discuss your survey responses if you're interested in tracking personal changes throughout the year. Our first survey is designed to gauge your overall wellness awareness, and as the year progresses, we'll provide opportunities for growth in various wellness categories.

6. Survey Process:

This quarter, we'll kick things off with a comprehensive survey to get an overall picture of where we stand.

Moving into Q2, our survey focus will narrow down to your Physical and Social Health. We want to see what positive changes or improvements you've noticed in these areas over the course of the activities we did in Q1.
